Maui Marriage License & Agents Information
Like all US locations, Hawai'i requires license for all marriages. On the island of Maui, the State of Hawai'i has authorized independent contractors to issue marriage licenses.
 
You will need an appointment with an authorized licensing agent to apply for your marriage license. This license may be used on any island and is valid for a period of 30 days. You and your spouse-to-be must attend the appointment together. Both parties must present a valid picture I. D. (drivers license or passport), and applicants 19 or under must have written parental or legal guardian permission.
 
The license fee is $65.00 payable in "cash only" directly to the licensing agent. This is your legal document. This document must be presented to the Minister/Officiant prior to ceremony. Your Minister/Officiant will ensure all is filled in & signed, then he/she will forward to the state for filing.
 
Your completed Marriage License will be mailed to you within 90-120 days. You may opt to receive your license earlier by filling out & mailing in an expedite form (which you will receive from your licensing agent at meeting).

You may fill in the License Application at home, however, use ONLY Black Ink, fill in completely, DO NOT SIGN. This will be signed in front of the licensing agent. DO NOT type this form.
 
The State of Hawaii does not require Blood tests. The State of Hawaii does require written proof If you were divorced less than 60 days of date of ceremony. Otherwise, just the date is required at time of application.
